The only sign that it was ever installed is a dimple on flat of shaft, from set screw. Anyway, since it was in the original box and so clean, I don't think its seen much if any use. I'm guessing that someone began to install the wrong motor for furnace or maybe technician misdiagnosed a bad motor, then put old back. There was no dust on the winding's or any trace of electrical smell that motors get when heavily used.
Looks unused, except for a set screw mark on the flat of shaft. There was light surface oxidation on the shaft, but it came off easily with emery paper.
Bench tested fine and ran smoothly on all terminals speeds. I repacked the motor in original box, but added a bunch more cardboard and foam padding. The motor is really clean.
